Sample Evaluation Forms

Evaluation of employees in the workplace is something that is a universal concept, however it is usually one of the things that is put t on the backburner for most employers. The employee evaluation or development plan is a tool that can take many forms however the basic idea is to review the overall performance of an employee and then make a plan for their continued development with the company and in their current position.
While the concept of employee evaluation is standard the methods used to evaluate employees varies greatly depending on the company or organization. Some employers choose to evaluate once a year while others choose to do this more frequently using many available sample evaluation forms. The general rule of thumb is that supervisors should be providing regular feedback to their employees so nothing that comes up on an annual evaluation is any surprise to the employee at the time of the review.

Some performance reviews are designed to provide feedback that will determine monetary compensation or promotion while others are simply tools to provide formal feedback. Regardless of the reason for the evaluation these should be taken seriously by both the supervisor and the employee. Sample evaluation forms are available to assist the supervisor in organizing the review.

 


The employee evaluation is otherwise known as a performance appraisal, employee development plan or annual review. The components of an employee evaluation usually includes a supervisory assessment on how the employee is doing in his or her current position, and evaluation of strengths, weaknesses, organizational goals and a development plan. The evaluation will then consist of the development of the goals and objectives that need to be met in order to for the employee to maintain their current position, be considered for promotion or advancement opportunities or to remain employed. The feedback is shared with the employee and goals and objectives should be discussed by the supervisor and employee.

A performance evaluation usually consists of three major components:

•An employee self-evaluation which allows for the employee to take a look at their strengths, areas that they feel they need to develop and long term goals for their employment.
•The supervisor’s performance review which includes ratings and feedback about the employee’s strengths, weaknesses and overall performance within the organization.
•An employee development plan which combines the goals of the employee, the areas that the supervisor has identified that need improvement and long term plans for employee development or possible promotion within the company.

These three components ensure that the process of evaluation is balanced and that all key areas have been taken into consideration and pave the way for active and productive discussion about the employee’s performance.

Here are two samples of an employee evaluation and employee development tools.

This sample uses a scale of 1-5 to assign a score to each area of employee work performance.

•Supervisor Name
•Review of Self-Evaluation
•If Applicable Review Goals from Last Evaluation:

Rate the Employee in each area of Job Responsibilities based on the following scoring key:

3= Meets Expectations
4=Frequently Exceeds Expectations
5=Consistently Exceeds Expectations

This sample provides general work performance areas and allows the supervisor to comment on each area of the employees work performance.
